  • Parish Registration Form:

Newcomers and long time residents are asked to complete a registration form for our records or to update the information we’ve got on file. It is our way to serve you better. If your phone number has changed or you’ve moved or are planning to move, please let us know. Please note that adult children should register independently once they complete their education and join the workforce, even if they continue to reside with you.  Download form here

  • Ministry Participation Form:

God has blessed each of us with many gifts and talents. To live in a Community of Faith we are called to share these gifts to strengthen the St. Agnes Parish Community. Your individual and unique talents may be just what we need. Our ministries are open to all confirmed parishioners 16 years of age and older unless otherwise noted. Training is provided as needed.  Download form here

  • Godparent/Sponsor Certificate for Parishioners:

You’ve been asked to be Godparent for your friend or family’s child. Please be aware a sponsor certificate is issued by the church where you are currently registered and attending Mass on a weekly basis. The St. Agnes policy states that you must be a registered parishioner for at least 6 months and be known to our priests. The sponsor certificate is not issued by the church where you made your sacraments if you no longer attend Mass and remain a member there. Download form here

  • Request Copies of Your Sacramental Certificates

For our parishioners who received sacraments at St. Agnes (Baptism, Communion, Confirmation or were married in the Church or at St. Francis Chapel) and require a copy of a sacramental certificate, please download the request form and return the completed form to the parish office either by email:  dona.st.agnes@sbcglobal.net or fax 860-691-2187.
